Stroke Risk Identification Program

 The Stroke Risk Identification Program is designed to: 1) identify individuals who are at high risk of developing stroke; 2) review and counsel with the participants their screening results, and
3) provide them with identified community medical resources for intervention treatment
 
 

 

Hospital Visitation Program

 The Hospital Visitation Program is designed to be a venue for disseminating stroke education among survivors and their caregiver during acute stroke care, for obtaining demographic data on stroke survivors, and to provide three-month follow-up telephone calls to recipients.

  Stroke can be treated for a more enjoyable life.
   
  We made it our goal to develop a three part community service program in stroke prevention, stroke education and post stroke care. With generous support, these programs are now in progress.
